You can download our new patient forms here, print and complete them, and either email them ahead of time to info@tjemmer.com or bring them to your appointment.
If you cannot complete them ahead of time, please come 15 minutes early to complete them prior to your visit with Dr. Emmer.

Where are you located?We are located in an office building at 310 Madison Avenue in Morristown New Jersey. We have plenty of parking. There are two parking lots with separate entrances. We are in Suite 230 which is on the 2nd floor and we have an elevator in our building. Our offices are handicap accessible.

What are your hours?Our office hours are Tuesday, Wednesday, Thursday from 8 am to 5 pm. Mondays and Fridays are reserved for special procedures. Extended hours and Saturday appointments are available on special request. Cleaning and exams are typically scheduled for Tuesdays, Wednesdays and Thursdays.

Do you take insurance?We are a non participating provider. However, we will help you file a claim with your insurance provider and we can develop financial plans to help you budget the cost of treatment as needed. Dr. Emmer has written a letter explaining why he has chosen not to participate with insurance carriers which you can read here.

What if I need to cancel an appointment?We understand that "life happens" and this may not always be possible due to sudden illness or changes in your busy schedule. Our policy, however, is to start taking a $50 non-refundable deposit should you re-schedule a broken appointment with less than 48 hours more than once. We ask you provide us with 48 hours (2 buisness days) notice if you need to change or cancel an appointment.

Does Dr. Emmer do implants?Yes, Dr. Emmer will help you understand if implants are an appropiate treatment option for you and he will work with you to develop a plan. He will also build your replacement teeth and recommend excellent endodontists and oral surgeons in the local area who can place your implant and/or perform any necessary treatments.

Do you charge for an initial consultation?Yes. We charge a fee of $225 for an initial consultation with Dr. Emmer. This fee covers the time Dr. Emmer and our office will spend with you to evaluate your condition and use our expertise and experience to develop a recommended treatment plan. Your recommended treatment plan may or may not include services to be rendered by Dr. Emmer in which case he will recommend providers who can help you. Should additional or repeat consultations be needed, these visits are not included with the initial fee.

What can I expect on my first visit?Please plan to arrive 15 minutes early to complete our new patient registration and agreement form and sign our privacy policy or download and complete those two forms ahead of time. The link to the forms can be found under "Patient Info." Dr. Emmer is considered a non participating provider with any insurance carriers. However, if you bring your insurance card with you, we will can assist to complete the forms on your behalf. Please be sure to understand your expected out-of-network benefits. We will happily review any diagnostic tools you have available, e.g. xrays, radiographs, study models, etc. Please have them sent to our office ahead of time. It is always helpful to have your referring dentist to give you a letter or call into our office ahead of time. Family or friends are welcome to attend with you during your initial consulation.
